Every new client signs one form before hands-on work begins. Completing it at home rather than in the treatment room means your appointment starts when you do. Find the one that matches your booking below — it takes a few minutes, and you only do it once.
Three Forms,
One Applies
If you are recovering from surgery — a BBL, an abdominoplasty, a facelift or rhinoplasty, or any procedure that left you with drains, incisions, or compression — complete the post-operative intake. If you booked Sculpt & Tone or wood therapy and have not had recent surgery, complete the body sculpting consent. If you booked shockwave therapy, complete the shockwave intake. Booked more than one? Fill in the post-operative intake first and mention the rest at your visit.
Post-Operative
Intake & Consent
For anyone booking drainage after surgery. It covers your surgery details, your current post-operative status — surgeon clearance, drains, compression, incision condition — your health history and medications, and what you want the work to address. It ends with a signature.

Body Sculpting
Consent & Waiver
For Sculpt & Tone and wood therapy clients. It explains what the treatment involves, records your medical history, and sets out the contraindications, the possible side effects, and the pre- and post-treatment instructions you are agreeing to follow. It ends with a signature.

Shockwave Therapy
Intake
For shockwave therapy clients. It records your medical history and the contraindications that would rule the treatment out, sets out your consent to treatment and the financial acknowledgement, and covers the pre- and post-treatment instructions. It also includes a photography release for social media — that section is optional, and declining it does not affect your treatment. It ends with a signature.
